CANADIAN ASSOCIATION OF OPTOMETRISTS

The Canadian Association of Optometrists, Harvard Western Insurance and Encon Group Ltd. have been
providing a Professional Liability Insurance Program for members of the Canadian
Association of Optometrists (CAO) since July 15, 2000. The program is designed to meet the
needs of Optometrists and gives the CAO members the insurance protection they require for
services rendered in Canada.

$5,000,000 Limit Annual Premium: $510.00 per year, per optometrist.
$3,000,000 Limit Annual Premium: $410.00 per year, per optometrist.
Policy period: July 15th, 2010 – 2011

Any member in good standing of the CAO will be automatically accepted for coverage if you have
had no claims. Anyone applying for insurance coverage who has had prior claims, will be subject
to review and you will be advised promptly regarding acceptance in the program. It is very
important that any claim or possible claim that you have knowledge of be reported to your
existing insurer now, before joining the program.
